Then a new page will open.

Now click on “Information on Online Applications” and then “Birth and Death Registration System” at the left side of the page

You will be directed to the new page.

Now click on the link “Birth and Death Application”

Again a new page will open



On this page, enter the Receipt Number and then click on submit button.

Then you will get the get the Online Registration form
