As you want to get ESIC Declaration Form 1 so for that you want to get process of getting this so here I am giving you same:

Firstly go on official website of ESIC.

On that page you have to go on Act section which is in the upper of page and click on ESI Act option.

And then on new page you have to do click on downloads option which is in the left side of page.

And then on new page you have to do click on FORM-1 Declaration Form option in the front of page.

And than you will get your form in a PDF file.
