UPCPMT admission procedure

I want to take UPCPMT exam so please give me the date of the exam and procedure of applying. I want to apply offline for the exam so describe the offline procedure or give me address of such any website from where I can read about it.

UPCPMT is an entrance examination conducted for admission of students into its diverse bachelor medical and dental courses in the medical and dental institutes in Uttar Pradesh by CSM Medical University, Lucknow.

Admission Procedure
Candidates are necessary to find the Information Brochure from any of the branches of Allahabad Bank by remitting the examination fee of Rs 1050.
Candidates belonging to the Sc/ST category can find the Brochure by paying Rs 550. An additional bank charge of Rs 30 will also have to be paid by the applicants.
The application form included with the booklet has to be filled according to the specifications mentioned. Accordingly filled in application forms are necessary to be submitted to the official address. Candidates are necessary to download the admit card from the UPCPMT official website.
The entrance examination will be 3 hours. It will contain objective type questions. The paper is divided into four parts: Botany, Zoology, Physics and Chemistry.
Every part contains 50 questions. The examination carries an entire of 200 marks.
Each question carries one mark and there will be judgment of marks for marking incorrect answer. The questions will be based on the 10+2 syllabus. Candidates are necessary to mark the answers in the OMR sheet provided.
Chosen candidates will have to concentrate the counseling section. The allotment of seat will be based on the marks available in the entrance examination.
