How can i apply for Chartered Accountant Common Proficiency test, or provide here registration application form???

Right now the Chartered Accountant Common Proficiency Test exam June dates are gone so you can not apply.
For your future reference Here is the procedure for applying Chartered Accountant Common Proficiency Test:

Go to the official website of Chartered Accountant Common Proficiency Test,
Click on Apply Online for CPT June, 2014
Click on How to Apply on-line for Exams option
Now click on new user,
Then login & get a application form
