How can I apply for UPTET exam, & how to get UPTET application form, will you please provide procedure???

Basic Education Council, Allahabad,Utter Predesh conducts Utter Pradesh Teachers Eligibility Test (UPTET).
Here are UPTET 2015 dates:

Date of Publishing of Advertisement: 10.12.2014

Last Date for Online Registration: 01.01.2015

Last Date for Online Application: 06.01.2015

Correction of errors:
01 to 16.01.2015

The Admit Card will be uploaded on the website:
06.02.2015

Date of Examination:
22 & 23 February, 2015

Answer Key will be issued on
25.02.2015

Examination Result:
27.03.2015

Right now UPTET application form is not available, next year in 2015, you’ll get UPTET application form from Basic Education Council, Allahabad,Utter Predesh official website.

I have filled the incorrect roll no. Of class12th.how can i get the solution?
