SCRA Part II Registration

As I have already complete my Part-I registration of Special Class Railway Apprenctices (SCRA) . Now I have to fill Part-II as per the instructions. Would you please provide me a link where I can complete my Part-II registration . Please guide me how to fill this form because I don’t have idea about it .
