NPS Account in PNB

My Father wants to open a National Pension System (NPS) account in Punjab National Bank, so he needs application form for PNB NPS, so will you please provide here???

All the new entrants, who have joined/ will join the bank on or after 01.04.2010, to join New Pension Scheme (NPS) and get them registered with NSDL immediately after their joining.

Required documents to submit with application:

Copy of Pan Card
Copy of residence Proof (voter I card/ Adhar Card / Ration card/
Driving Licence/ any other proof)
One Cancelled Cheque
