How to calculate the Cut Off for Paramedical Courses

One of my relative is completed his 12th standard now.

1) For paramedical courses how medical board is going to calculate the Cut off either only considering 12th or both 11th & 12th. ??? (Please provide the calculation method)

2) His Category is BC. How much cut off mark should achieve to get seat in Government Medical college for Paramedical courses like B.Sc Cardiac/Anesthesia/Radiology ???

3) When the application forms will issue in medical colleges ???
