As per my knowledge Board of Technical Education, Delhi conducts a Common Entrance Tests for admission to various courses in Delhi Polytechnics on behalf of Department of Traning and Technical Education, Government of National Capital Territory of Delhi, and here you are asking for dates of form for the same exam so I am telling you about date:

Submission of online Application Forms : 07-04-2014

Closing date for online Application Forms : 28-04-2014

Date of CET Test are

Date for Test – I: 08-06-2014 10:00am to 12:30pm
Date for Test – II: 08-06-2014 02:00pm to 04:30pm
Date for Test – III: 07-06-2014 10:00am to 12:30pm
Date for Test – IV: 08-06-2014 02:00pm to 04:30pm
Date for Test – V: 07-06-2014 10:00am to 12:30pm

As you can see that all dates are gone so you can try for it next year:

The Department of Training & Technical Education (DTTE) conduct the Delhi Polytechnic Common entrance test (Delhi CET 2015) for candidates who want to take admission into the diploma courses

Submission of application form through debit card / credit card / net banking / challan starts from:-

CET examination date:-
June 2015

Eligibility:
Candidate must passed 10th from recognized board to apply in this entrance examination.

Age Limit:
Students must have the age above of 15 years.

Exam Pattern:
Question paper consist of only MCQ questions

There are 120 questions
Mathematics: 60 Question
Chemistry: 30 Question
Physics: 30 Question
