Firstly go on official website of DA-IICT Centre for Entrepreneurship and Incubation.

On that page upper side you have to click on Incubation section.

On new page in the left side you have to do click on Apply for Incubation option.

On new page you have to do click on Download option in the front of page.

And then you will get applying form in a PDF file.
