Calicut university duplicate certificate

I have passed my MCA from the Calicut university I have lost my MCA original Mark list? I want to apply for the Calicut university MCA duplicate Mark list? So can you please tell me from where I can get the Application form for Duplicate Mark list?

The Calicut University is provides duplicate certificate after filling application form. After filling application form will be send by candidates to address of Calicut University.

The University of Calicut is the largest University in Kerala. Established in the year 1968, it is the second university to be set up in Kerala.

The University was created through a Government plan bifurcating Kerala University.

Application for duplicate certificate of Calicut University





Fee Rs. 250/- per mark list + postal charge
Rs.50/- for the duplicate certificate the fee is Rs. 350/- (including postal charge) up to 5 years from the date of passing the degree.

After 5 years fee for duplicate certificate is Rs.450/- and after 10 years the fee is Rs. 550/-

Fee for obtaining duplicate degree/diploma certificate for second time is Rs 1250/--with the application form news paper (in full) should be enclosed with the application for duplicate Pre-degree mark list, attested Photocopy of SSLC 1 st page also should be enclosed.

Affidavit
Chalan receipt (original)
Copy of lost marklist (if available)
