I have done graduation degree from Indian Institute of Technology Guwahati and I want the application form to issue the transcript so will you please provide me? Can I download this form from the official website of the Indian Institute of Technology Guwahati, if yes then provide me the process of it?

Then click on tab Forms Section Given in right site

Click on General forms

Then a new page will open look like image
