GSPC GAS Connection

I want to take a new connection in GSPC GAS so tell me what the charges to take new connection are and where I can get the application form?

If you want to apply for a new GAS connection then you should fill the application form and have to provide a photocopy of any two of the following

Passport
Ration Card
Most recent house tax bill
Certificate from the Society
Most recent electricity bill
Most recent telephone bill (BSNL)
