BPUT Paper Rechecking

I am not satisfied with the MBA first semester result of Biju Patnaik University of Technology (BPUT) and now I want to fill the rechecking form for revaluation in 2 papers of MBA first semester exam . From where I can get the BPUT rechecking form for revaluation?

I sudeepta patra of Padmanava College Of Engineering,Rourkele.My issue is that my 3rd sem rechecking result was not showing.so ,I request that please recheck my paper and show the result....
