TET Application Form UP

I am interested to apply to UPTET but I have no application form so can I know from you where I get UPTET Application Form?

You want application form for the Uttar Pradesh Teachers eligibilities test.
State Council of Educational Research & Training Uttar Pradesh (Uttar Pradesh Government) has published a notification that Teachers Eligibility Test (TET) 2012 will be organized in Uttar Pradesh for recruitment of 72,825 teachers in Uttar Pradesh UPTET stay till 30-4-2012.

Now the exam has passed so I can’t upload here application form for UPTET.
When the utter Pradesh publish recruitment for the Teachers eligibilities test then I will provide you application for it.
