I wish to apply for LDC exam which held by Staff Selection Commission so can you suggest me where I can download its application form and how to download?

SSC published the recruitment notification for LDC. The last date of submitting the application form was on 16-09-2011. The exam was held on 04-12-2011. The exam is over but you are asking for the application from so here I am uploading a file that contains the SSC LDC application form. You can download it from here.
