SBI IRCTC Card Status

Three months ago I have submitted application for the SBI IRCTC card but I have not received the card yet so want to see the status of my form. Can I check the status? If yes, please give me the website and also describe the procedure of it.

After clicking on that link, it will display a page which looks like the image given below. On that link first of all you will enter your date of birth, then enter your application no, then enter your form no and then click on submit button.
