You want to get format of Know Your Customer at Reserve Bank of India so here it is:

NBFCs were advised to follow certain customer identification procedure for opening of accounts and monitoring transactions of a suspicious nature for the purpose of reporting it to appropriate authority. These ‘Know Your Customer’ guidelines have been revisited in the context of the Recommendations made by the Financial Action Task Force (FATF) on Anti Money Laundering (AML) standards and on Combating Financing of Terrorism (CFT). Detailed guidelines based on the Recommendations of the Financial Action Task Force and the paper issued on Customer Due Diligence (CDD) for NBFCs by the Basel Committee on Banking Supervision, with indicative suggestions wherever considered necessary, have been issued. NBFCs have been advised to ensure that a proper policy framework on ‘Know Your Customer’ and Anti-Money Laundering measures with the approval of the Board is formulated and put in place.

2. This Master Circular aims at consolidating all the instructions/guidelines issued by RBI on Know Your Customer (KYC) norms/Anti-Money Laundering (AML) standards/Combating Financing of Terrorism (CFT)/Obligations of NBFCs under PMLA, 2002. The Master Circular has been placed on the RBI website.

KYC means “Know Your Customer”.

It is a process by which banks gets information about the identity and address of the customers. This process helps to ensure that banks’ services are not misused. The KYC procedure is to be completed by the banks while opening accounts.

The RBI KYC format with basic details is as mentioned below:

Know Your Customer (KYC) Form in respect of the non-resident investor
Registered Name of the Remitter / Investor
(Name, if the investor is an Individual)

Registration Number (Unique Identification
Number* in case remitter is an Individual)

Registered Address (Permanent Address if
remitter Individual)

Name of the Remitter’s Bank

Remitter’s Bank Account No.

Period of banking relationship with the remitter

Note* Passport No., Social Security No, or any Unique No. certifying the bonafides of the remitter as prevalent in the remitter’s country

We confirm that all the information furnished above is true and accurate as provided by the overseas remitting bank of the non-resident investor.
