PMT Entrance Exam Forms

I want to apply for the UP Combined Pre Medical Test Entrance Exam but I have no idea about the forms so please can you give me the process of getting the application forms and tell me the date of availability of forms?

The application form to apply for UP Combined Pre Medical Test Entrance Exam can be downloaded from the official website of the university.
The Application form can also be obtained from all major branches of Allahabad Bank in Uttar Pradesh by paying Rs. 1050/- (Rs. 550/- for SC/ST) + Rs. 30 (Bank Charges) in cash.

The application form will be available from 19th April , 2012.

then you have to do click on PROCEED TO APPLY ONLINE link from that link you will proceed to get form. Form currently not available.

Steps to Apply Online:

Download the Information Bulletin and read it to ensure your eligibility

Fill the Online Application Form and note down the Registration Number

Upload photo, signature, Left Hand Thumb Impression and Self Declaration

Make Payment of Fee through Credit Card/Debit Card or by e-challan

Print Confirmation Page after Successful remittance of fee .
