The New Jersey State Police (NJSP) is the state police force for the U.S. state of New Jersey.

The 212A Form should be used only after consultation with your local Police Department or the State Agency that is responsible for the type of licensing needed.

You must first obtain an Originating Agency Identifier (ORI) Number from the licensing department to complete this form. Incorrect ORI Number may result in non-processing of your form.

Each submission costs $20.00.

Purpose of this form:

Form 212A is used exclusively for the purpose of local Police Department and State Agencies, to obtain background information for the purposes of employment, licensing and local ordinances
