As you want to get the membership rates and the application form of membership of National Council for Black Studies so here is the information of the same for you:

Membership Rates:
Institutional Membership status is reserved for actual academic institutions of any level. $300/year

Professional Membership includes faculty or staff from any academic institution. $100/year

Affiliate Organization Membership includes non-academic organizations of any level, such as churches, professional associations, etc. $200/year

Associate Membership includes individuals who are not faculty or staff of an academic institution. $100/year

Student Membership includes anyone currently enrolled in an academic institution. $35/year

Senior Citizen Membership includes anyone 65+ who is not full-time faculty or staff at an academic institution. $50/year

Individual Life Membership for individuals who wishes to make a substantial monetary contribution to NCBS. $1,500 or 3 years @$500
