National eligibility test Challan form

my friend is filling the application form to give the National eligibility test which is conducted by the University Grant Commission. I am looking for the Challan form of it. so will you please give me the Challan form of National eligibility test? give me the details of the fee of this exams?

Age Criteria:

Applicant must have age limit is 30 years

Qualification Requirement:

Candidates who possess Master’s degree or equivalent from a university recognized by UGC can apply.
D candidates can also apply.
It is necessary to have at least 55% marks in aggregate in the master’s degree. 50% marks for reserved categories.
