KUK University DMC

I am pursuing B.Com from Kurukshetra University, Kurukshetra (KUK) but I have lost my 3rd semester DMC and now I need duplicate DMC for interview in government job. Would you please tell me what should I do? Will I have to submit Application for it? If yes, please provide me Application Form for issuing of Duplicate DMC.
