Details:

The HealthWatch Support Specialist is responsible for identifying eligible candidates for Disease Management (DM) and conducting initial outreach calls for program enrollment; the appropriate triage of calls for GPA Nurse Navigator (NN) and for providing program support to coordinate indicated medical services.

Qualifications:

To perform this job successfully, an individual must be able to maintain a high level of accuracy and attention to detail to perform each essential duty satisfactorily.

Required Experience:

High school diploma or equivalent is required. Previous experience in handling customer queries, providing excellent customer service, and communicating well verbally and in writing.
