Mark sheets of Degree Part -I & II (TDC) and Part -II & III in case of (1+1+1) Examinations must be enclosed.

* Required filling are mandatory.

An attested copy of Final Admit Card, University Registration Receipt and Final Mark sheet of Degree final examination must be enclosed.

In case of M.B.B.S. examination, an attested copy of Admit Card, University Registration Receipt, Marksheet and Internship Completion Certificate must be enclosed.
