The MCH Neurosurgery is a 3 Years Course offered by AIIMS Delhi.

TRAINING SYSTEM
Exclusively on whole time in service basis, on residency pattern.

ELIGIBILITY CRITERIA
For Direct 6 year course
M.B.B.S. degree of an Indian University recognized by the Medical Council of India or any other equivalent degree recognized by MCI.

During the MBBS course the candidate should have atleast 2 weeks rotation
in Neurosurgery and the Neurosurgery related teaching should be taught by qualified Neurosurgeon wherever available.

MODE OF SELECTION:
AIIMS takes admissions twice in a year for MCH Neurosurgery Course.

TRAINING METHODS
Clinical teaching in the OPD, Emergency and Operation theatres. Clinical teaching rounds in
Neurosurgery Ward and bed side presentations.
Seminars, journal clubs, mortality, morbdility conferences.
Assisting and performing neurosurgical operations.
Paper presentations at conferences.
Training in an experimental microsurgical laboratory
Treatment planning sessions.
Preparation of manuscript for publication.
Special teaching sessions like Neuroradiology rounds, Neuro-ophthalmalogy round combined
Neurology-Neurosurgery case discussions.

Course Contents

Clinical Neurosurgery including history taking, physical examination, diagnosis, selection and planning of relevant investigations, appropriate treatment and rehabilitation of patients with neurosurgical disorders including those presenting as emergencies

Essentials of clinical Neurology especially with reference to disorders common in India and those likely to present to the Neurosurgeons.

Basic medical sciences relevant to the practice of Neurosurgery.

Surgical Neuropathology and the essentials of the Pathology of Neurological disorders likely to present to the Neurosurgeon.

Performance and interpretation of Neuroradiological procedures, such as carotid arteriography and myelography. Familiarity with the technique of selective arteriography and its interpretation.

Principles and interpretation of common Neurophysiological, Neuro-ophthalmological, Neurootological and Neuroendocrinological tests especially with reference to Neurosurgical disorders.

Principles and interpretation of computerized axial tomography, MRI and other modern investigations.

Performance of common neurosurgical operations in the supra and infra-tentorial compartments in the spinal canal and on the peripheral nerves – initially under supervision and later independently.

Ability to use the operating microscope is mandatory.

Familiarity with various types of anaesthesia used in neurosurgery their indications and
contraindications, the use of ventilators and techniques of monitoring and resuscitation.

Pharmacology of various drugs used in Neurosurgery.

Knowledge of the history of neurological surgery and its allied disciplines with special reference to India.

Knowledge of recent advances in the field of neurological surgery.

Preparation of papers for presentation at scientific conferences and for publication.

Introduction to the techniques involved in the organisation and development of a department, its subsections and newer facilities.

It is desirable to have microsurgical laboratory training where candidates learn dissection/suturing of fine arteries/nerves under microscope and skull base dissections.

Development of proper attitudes towards patients, subordinates, colleagues and seniors.

Should have basic knowledge about application of computers.

Neuroradiology
Combined Neuroradiology rounds or meetings twice or thrice a week

EVALUATION OF M Ch (NEUROSURGERY)
Internal assessment – 20% weightage
Theory Papers – 30% weightage

Timing of Examinations
Part I – at the end of 2 yrs. for 6 yrs. course on General Principles of General Surgery and basic neurosciences.

Part II – at the end of 36 months of training (for post MS) and at the end of 6 yrs.

Practical Examinations
Total weightage 50%
Distributed as follows:
a) Clinical - 20%
b) Operative demonstration for M Ch - 20%
c) Radiology, Pathology and general viva - 10%
Minimum pass marks 50%

Department of Neurosurgery in AIIMS (All India Institute of Medical Sciences), New Delhi started a humble beginning in March 1965. It is equipped with ultra-modern technology and equipments from time to time i.e. operating microscope, Laser, CUSA, Ultrasound, Intra-operative MRI, Gamma Knife and Image Guidance System, which are necessary to provide state of the art care at par with the best neurosurgical centres anywhere in the world.


MCh (Neurosurgery)

Duration


There are two streams of neurosurgery course in the department
1. 3 year course
2. 6 year course


Eligibility

1. 3 year course: - One must possess M S (General Surgery) degree of a University recognized by Medical Council of India. The entrance exam is conducted in December and May every year.

2. 6 year course: - One must possess MBS degree of a University recognized by Medical Council of India and must have completed the required period of pre-registration internship (12 months) in a recognized hospital. The entrance exam is conducted every May and December.


Procedure of Selection


1. 3 year course: -

One is selected through 2 step competitive exam held at national level.

Step-I Written exam of multiple choice questions based on MS (General Surgery) and Basic Neurosurgery. The candidates are short listed for step-II.

Step-II There is a departmental interview where candidates are examined to evaluate their clinical acumen and aptitude.

Final selection of the candidate is made on the bases of combined evaluation of step-I, and II


2. 6 year course: -
One who clears the AIIMS post graduate entrance exam after MBBS are called for counselling and the selection is made on merit base.


Course

3 Year Course


One selected in 3 year stream works as Senior Residents in the department.
One is supposed to do all clinical work of wards, ICU’s, emergency and assist cases in operation theatres under supervision.
One is given increasing responsibility for independent surgical work.
One is expected to maintain a log book of all surgical experience.
One is supposed to write two dissertations (one prospective and one retrospective study) to qualify to appear in final MCh exam. The final MCh examination to award the degree takes place after the end of 3rd year.


6 Year Course

One selected in 6 year MCh course, spend first 3 years as Junior Residents. During this period one is posted for 1 year in General Surgery on rotation.
After completing 3 years, one works as Senior Residents in the department for next 3 years. During these 3 years one has similar responsibilities as that of a senior resident from the 3 year stream.
One also has to write 2 dissertations (1 prospective and 1 retrospective study) to qualify to appear in final MCh exam.
One has to appear for the final MCh exam after the end of 5 years. During the 6th year one works in the department as chief residents.

c. Two short cases
d. Radiology viva
e. Pathology specimens viva
f. Grand Viva
