Firstly go to the official website of Queen’s Faculty of Law which looks like this image:



Now take your cursor “JD Admission” option and click on “Apply” option a new page will open which look like this image:



After that right side of the home page you will see “Applicant forms” option and click on that option a new page will open which look like this image:



After that click on “Common Law/Civil Law Application Form” option a pdf file will come here I am providing you application form in image format.
