Application/Admission Requirements
Applicants should have Original and Photocopy/CTC of Transcript of Records (from last school attended)
Applicants should have Original and Photocopy of Certificate of Marriage (for female married applicant only)
Applicants should have Original and Photocopy of Birth Certificate (NSO Authenticated)

Original copy of the following:
Applicants should have Certificate of Employment or Service Record (if employed)
Applicants should have Certificate of Good Moral Character (from employer) or Brgy. Certificate/Clearance
Medical Certificate



Admission Prerequisites
Bachelor's Degree –
Appropriate or related to the program; if the bachelor’s degree is in another field, the student has to make up for deficiencies by taking 18 units of pre-degree courses prescribed in the relevant program of studies in this University.

Grade Requirement –
Applicants should have scholastic rating of at least 2.0, B, 85% or better in the undergraduate courses

Applicants should have Qualify in the admission examination

Applicants should have Pass the interview by the Program Chair and/or the Dean
