5. CRRI Completion Certificate
6. MBBS Degree / Provisional Pass Certificate issued by the University
7. Permanent Medical Council Registration Certificate
8. Course Certificate from the Head of the Parent Institution for MBBS
programme
9. Copy of the orders of MCI recognition of the College for MBBS, in case of
candidates who passed MBBS from Colleges in other states
10. 3 Numbers of Passport size photographs (background – white/blue)
