Procedure To Get Migration Certificate From EIILM University

I am student of Eastern Institute for Integrated Learning in Management University (EIILM) and want the migration certificate. Will you please tell me procedure to get Migration Certificate from EIILM University and also if possible provide me Migration Certificate form in PDF?

For Issue of Provisional / Migration Certificate

You may apply for the issue of Provisional Degree Certificate / Migration Certificate and send the documents to University.
1.Application form
2.Copy of all marksheets issued by the University
3.Fee of Rs.1000/- per certificate
