PQRST Method of Pain Assessment Hints and kinds:

P = Provokes
What causes pain?
What makes it better?
Worse?

Q = Quality
What does it feel like?
Is it sharp?
Dull?
Stabbing?
Burning?
Crushing? ( Try to let patient describe the pain, sometimes they say what they think you would like to hear. )

R = Radiates
Where does the pain radiate?
Is it in one place?
Does it go anywhere else?
Did it start elsewhere and now localised to one spot?

S = Severity
How severe is the pain on a scale of 1 - 10?

T = Time
Time pain started?
How long did it last?



Characteristics of pain (PQRST) :

(1) Palliative / Provocative factors of pain
(2) Quality of pain
(3) Radiation (Location) of pain
(4) Severity of pain
(5) Time of pain
(6) Associated phenomena

The PQRST Method of Assessing Pain is a Valuable tool to Accurately Describe, Assess and Document a Patient's Pain.


The method also aids in the selection of appropriate pain medication and evaluating the response to treatment.

OPQRST is an Mnemonic used by Medical Professionals to Accurately Discern Reasons for a Patient's Symptoms and History in the Event of an Acute Illness.

"PQRST" Stands for :
P = Provokes
Q = Quality
R = Radiates
S = Severity
T = Time

Quality of Pain Management

Effectiveness of pain relief
Minimization of side-effects
Minimization of complications
