From where can I obtain application form for MP DMAT exam?

You want to get application form for MP DMAT exam so application forms for entrance examination will be available at the selected branches of State Bank of India on payment of 750/- through cash.

MP DMAT application Forms are also available at all participating colleges/ Association Office on payment of 750/- through cash / demand draft in favour of APDMC payable at Bhopal and its also can downloadable from official site of APDMCMP. In case the application form has been downloaded from website it shall be submitted alongwith Demand Draft of 1500/- and drawn in favour of “APDMC”, payable at Bhopal.


MP DMAT application forms should be submitted to the following address by Registered / Speed Post / Courier / in person with Examination Fee of 750/- by Demand Draft drawn in favour of “APDMC”, payable at Bhopal. Candidates depositing before Second Week of July, 2013 and Second Week of July, 2013 with late fees of 200/-.
