Migration Form Of PTU

Hello sir I want the migration form of the PTU (Punjab technical university) because I am going to shift at Chennai and want to take admission at other University of Chennai. Can you please tell me how to get the migration form of the PTU University. Is there any online mode to get migration form?

Migrations have to be applied only on the prescribed form to obtain.

Migration within the same city is not allowed. In case of genuine reason, special cases may be considered subject to the approval of the University through RC.

The migration application is to be submitted with the RC at least 60 days before the commencement of exams so as to be put-up the University through RC 45 days before the commencement of exams.

Migration shall be allowed after completion of the second semester but before start of the 3rd semester. The application should be submitted to the University within one month of start of session along with NOCs from the both colleges
