Migration Certificate from PTU

I have done BBA from the PTU last year. I want to take admission in other University for MCA Program. So I want Migration Certificate of BBA Course. So tell me how I can get that Certificate? Is there any application form to apply for Migration Certificate, so pls provide it to me?
