Submit the printed form with following documents:

Original certified and legalized copies of undergraduate academic certificate (1 copy)

Original certified and legalized copies of undergraduate academic transcript (1 copy)

Degree equalization from DIKTI (for overseas graduates)

Recent color photographs with blue background sized 4X6 cm (3 copies)

A health statement letter issued by professional medical examiner (medical doctor)
