Firstly go to the official site of the Jammu University.

Then at the right of yours there is a tab of downloads.

Click on that.

After that a new page will open that looks like the given image.



Then on that page at no. 46 there is a tab of Application Form for B.Sc. Nursing for the session 2011-2012.

Click on that, then a PDF file will open which contains all the requirements regarding to the form.
