You are looking for the Indian Forest Service Recruitment Application Form but as on today 18th Jan 2013, the UPSC does not publish the recruitment notification for the IFS, the UPSC published the IFS notification in the year 2012 April.
But for your tentative idea here I am giving you the info required for UPSC IFS:

UPSC IFS:

The applicant should have a bachelor’s degree with at least one of the subjects namely Animal Husbandry & Veterinary Science, Botany, Chemistry, Geology, Mathematics, Physics, Statistics and Zoology or a Bachelor’s degree in Agriculture, Forestry or in Engineering, from any recognised university.

Selection Process:
The eligible candidates will be selected on the basis of performance in Written Test and Interview/Personality Test.
