Here is the procedure for getting Institute of Company Secretaries of India application form:

Go to the official website of Institute of Company Secretaries of India
Click on student option given at the right side of page
Click on “Forms For Students” option given at the right side of page
Click on Examination option given in student section
Click on “Verification of Marks of CS Examination – Procedure/ Application Form”
Then download that
