Two self-addressed stamped envelopes of size 4” X 9” each with postal stamps worth Rs. 40/- affixed
Duly filled-in and signed Provisional Admit Card 2016 (with photograph pasted)
Duly signed copy of Online Supplementary Application Form (OSAF) 2016
Duly-filled in and signed Primary Application Form 2016 (with photograph pasted)
Duly-signed copy of Rules & Regulations 2016
One Attested copy of document providing proof of date of birth (Admit Card / Pass Certificate of Class X)
One Attested copy of Scorecard of any Aptitude Test (CAT / XAT / MAT / CMAT / ATMA), not more than one year old
One Attested copy of Community / Caste Certificate in case of SC / ST / OBC-I / OBC-II / BPL
One Attested copy of Residential Certificate, in case the candidate is availing benefit of reservation
One Attested copy of Income Certificate in case of SC / ST / BPL (within six months from the date of this application)
One Attested copy of proofs of belonging to the BPL category (Ration Card + BPL Certificate), if applicable
One Attested copy of Class X mark sheet
