Although I am giving information related to ‘Supply of medicines / drugs, surgical items, sutures etc. to HP Govt. hospitals and other Health institutions for the year 2015-16’ for your reference:

i) The tenderer should be manufacturer having valid manufacturing license or direct importer holding valid import license of quoted medicines/drugs, sutures, surgical items. If the product is imported, an attested photocopy of valid/renewed import license in Form 10 with Form 41 (as per Rule 122A of Drugs & Cosmetics Act) is required to be submitted.

ii) The tenderer quoting rates for the medicines/drugs, sutures, surgical items should have minimum annual turnover of INR 5 Crores during the financial year 2010-11, 2011-12 and 2012-13 in medicines/drugs, sutures, surgical items duly authenticated by a Chartered Accountant based on the Balance Sheets, Profit and Loss Account of the tenderer as per prescribed Format at Annexure “P1”.

iii) The tenderer should have at least have 3 years Market Standing as a manufacturer/importer for each drug quoted in the tender as a manufacturer/importer.

iv) The tenderer has to submit cost of tender document in the shape of cash/ bank draft/RTGS.

v) The tenderer has to submit Earnest Money Deposit (EMD) as required vide Clause No. 2 (i) & (ii) in the shape of Bank Draft/RTGS.

vi) The tenderer has to submit and upload Annexure “P10” i.e. Form for scrutiny of tender document duly filled in, signed and complete in all respects furnishing all the required information and documents with Page No.’s.The sample document is enclosed as Annexure “P11”.

vii) The tenderer shall have the average monthly production capacity not less than one and half times of the quantity earmarked for HPSCSC for each drug. However, after evaluation of the price bid, the eligible tenderers may revise the quoted quantity for a particular drug and the production capacity earmarked for such drug, provided, the total production capacity earmarked on such revision shall not exceed the sum of originally earmarked production capacity for all the quoted items. However, downward revision of the quoted quantity and/or production capacity is not
permitted. HPSCSC Ltd. has the prerogative either to accept or reject the revision given by the tenderer among the items.

viii) The tenderer has to submit and upload instruments such as Power of Attorney, Resolution of Board of Director etc. authorizing an officer of the tenderer to transact the business with Tender Inviting Authority in Technical Bid and such authorized officer of the tenderer should sign the Tender documents.

ix) The tenderer has to submit and upload Authorization letter nominating a responsible person of the tenderer to transact the business with the Tender Inviting Authority with duly attested signature & photograph.

x) The tenderer has accepted and signed Form “A” and Form “B” of the tender.

xi) The tenderer hasto submit and upload Turnover Certificate for the financial year 2010-11, 2011-12 and 2012-2013 duly authenticated by a Chartered Accountant as per format at Annexure “P 1”.

xii) The tenderer has to submit & upload valid GMP (GOOD MANUFACTURING PRACTICES) certificate(s) as laid down under revised schedule - “M”, or WHO-GMP/COPP (for importer) of the Drugs and Cosmetics Rules issued by Licensing Authority.

xiii) The tenderer has to submit& upload valid ‘GLP’ (GOOD LABORATORY PRACTICES) certificate(s) as per schedule L1 of the Drugs & Cosmetic Rules.

xiv) The tenderer has to submit & upload attested photocopy of valid Drug Manufacturing License(s),Import License(s) against which products have been quoted.

xv) The tenderer has to submit & upload attested photocopy of product license(s) from concerned Drug licensing authority in respect of quoted products.
