How can i get Company Secretary application form please provide procedure & important dates?

The Institute of Company Secretaries of India conducts Company Secretaryship Course is held in three parts:
Foundation Programme,
Executive Programme and
Professional Programme

Eligibility:
The applicant should have cleared 10+2 Exams.
Provisional Registration is also allowed for students who have appeared for their 10+2 Exams and are awaiting their Results.

Important dates:
December Exams Last Date for Registration
31st March 2014

Application form:
Applicants can get CS application form from ICSI official website.

Now the date has over, but for idea i am giving the procedure for getting CS application form:

Go to the official website of ICSI,
Click on the button “ONLINE SERVICES” on the top of ICSI website.
Click on “Student Registration”
Click on the checklist of documents as applicable on the basis of qualification.
Click on the link “PROCEED TO REGISTRATION”
Fill all required information & click to register
