Bangalore University Migration Form

I have completed MBA degree from the Bangalore university and want to get the migration certificate of it so can you please provide me the application form to obtain the migration certificate from Bangalore university or give me the process from where can I get this?

Sir pg admission for rtmnu and conform my admission but no migration certificate so please help me
