Requirements for new students to enroll in Azle schools:
Proof of Residency -- must be one of the following:
Electric bill
Water Bill
Lease Agreement or House Contract
Up-to-Date Immunization (Shot) Records
Birth Certificate
Copy of Student's Social Security Card
Grades/Credits from Former School (High School)
Withdrawal form from last school (for students transferring from another school during the school year.)
Transcript (10th - 12th grade students)
Last Report Card (9th grade students)
Copy of Parent's Driver License
