For Last Date of Submission of Challan in Axis Bank:

Without Late Fee: 23rd November 2015
With late Fee Rs.500/-: 08th December 2015
With late Fee Rs.1000/-: 07th January 2016

For Last Date of Submission of Print copy of form and a Copy of Bank Challan in DDE Office:
